    How many forms does a student need to submit for sports?

    Three forms – Manhasset Public Schools Health Appraisal Form (A), submitted once a year, a Health History Form (B), and Athletic Department Code of Behavior (C), completed by parents and students each season.

     How often does a student need to have a physical exam/medical evaluation to participate in sports?

    All athletes must have a sports physical exam/medical evaluation after

                May 31, 2018 to be valid for the entire 2018-2019 school year.

     Does a student need to have a physical by the school doctor for sports?

    No, they may have a sports physical exam/medical evaluation by their private health care provider. They may use the Manhasset Public Schools Health Appraisal Form (A), or the form provided by their Doctor.

    This form must be completed and signed by the parent and physician.

     When can a student have a school sport physical?

    The school doctor is available for sport physicals on the following dates:

    Thursday, June 7 from 3:15 – 5 pm

    Thursday, August 9 from 9 am – 10:30 am

             Wednesday, September 5 from 3:15 pm – 5pm.

     What does a student need to bring with him/her to have a physical with the School Physician?

    The student needs a copy of the Manhasset Public Schools Health Appraisal Form (A), which can be picked up in the Health Office, Athletic Office, or downloaded from the Manhasset website.

     What is the Health History (B)/Code of Behavior(C) form?

    This form must be completed for each sport in which the student participates. It must be dated within 30 days of the start of the sport. This year it should be dated after 7/23/18 for football and cheerleading, and 7/30/18 for all other V/JV fall sports. Middle School fall forms should be dated after 8/6/18.

     If my son/daughter played soccer, does he/she need to submit another form for lacrosse?

    Yes. For every season and individual sport your child tries out – he/she will need a Health History/Code of Behavior form.

     What if my child does not have the appropriate forms by the first day of sport tryouts?

    Your child will not be cleared for participation.

    What if my child had a medical note on file in the Health Office in June 2018 stating that he/she was not allowed to participate in physical education and/or sports?

    Your child will NOT be allowed to participate in August 2018 unless he/she submits a health care provider note which specifically addresses the reason your child was not allowed to participate and which now fully allows them to participate in “all sports and physical education.” If this note is not submitted to the Health Office before the beginning of sports/school, your child will not be cleared to play/participate.

     When do high school sports begin?

    High School fall sports always begin in August. For Fall 2018: JV/Varsity Football and V Cheerleading begin Monday, August 20. Boys/girls crew, boys/girls cross country, girls field hockey, boys/girls soccer, girls swimming and diving, girls tennis, boys/girls volleyball and boys badminton begin Monday, August 27, 2018.
